Tier 2

More than $141,000 up to $176,000 (individual MAGI)

2027 tier is a projection. CMS has not announced 2027 dollar amounts yet — the amounts below are 2026 official rates, shown as a placeholder. Expect 2027 amounts to be higher.

Part B surcharge (2026 rates)
$202.90/mo
$2,434.80/yr
Part D surcharge (2026 rates)
$37.50/mo
$450.00/yr
Total IRMAA per person (2026 rates)
$240.40/mo
$2,884.80/yr

$26,001 more 2025 MAGI (reaching $176,001) moves you up a tier and adds $1,735.20 per year in surcharges per Medicare enrollee (at 2026 rates).

Part D IRMAA is paid in addition to your plan premium. A Part D surcharge applies only if you have Part D (or a Medicare Advantage plan with drug coverage).

2027 IRMAA brackets — projected

Not yet official. Thresholds are computed with the inflation formula in the Social Security Act; dollar surcharges for 2027 are not yet announced, so 2026 amounts are shown as placeholders.

Thresholds are our projection from the statutory CPI-U formula; tiers 2–4 may land $1,000 higher (single) or $2,000 higher (joint), shown in parentheses. How we projected these.

IRMAA 2027 brackets for married filing jointly

For joint filers, every projected 2027 threshold is exactly double the single threshold. Based on 2025 joint MAGI, the 2027 surcharge is projected to start above $224,000 (2026: $218,000). The next tiers start above $282,000 (or $284,000), $352,000 (or $354,000), $422,000 (or $424,000), and at $750,000 — the top joint tier is frozen by law through 2027.

IRMAA is charged per person: if both spouses have Medicare, each pays the surcharge for the tier set by the shared joint MAGI, so crossing a threshold by $1 raises two premiums. See jointly vs separately.

What crossing each IRMAA cliff costs

Each threshold is a cliff: $1 over moves you into the next tier for the whole year. Cost per Medicare enrollee at 2026 official rates (2027 rates not yet announced):

Worked examples for 2027

Single, $115,000 MAGI in 2025

Projected 2027 result: Tier 1 (More than $112,000 up to $141,000).

At 2026 rates: $95.70/month per person ($1,148.40/year).

Married filing jointly, $280,000 MAGI in 2025

Projected 2027 result: Tier 1 (More than $224,000 up to $282,000 joint).

At 2026 rates: $95.70/month per person ($1,148.40/year); $2,296.80/year if both spouses have Medicare.

Note: $280,000 is just under the projected $282,000–$284,000 tier-2 line — a small amount of extra income could change the tier.

Single, $108,000 MAGI + $10,000 Roth conversion

Projected 2027 result: Tier 1 (More than $112,000 up to $141,000).

At 2026 rates: $95.70/month per person ($1,148.40/year).

Without the conversion ($108,000) there would be no 2027 IRMAA; the $10,000 conversion crosses the $112,000 line and costs about $1,148 in extra premiums at 2026 rates.

How the 2027 projection is calculated

Under 42 U.S.C. §1395r(i)(5) (Social Security Act §1839(i)(5)), each IRMAA dollar threshold is increased by the percentage rise in the average CPI-U (all urban consumers, U.S. city average) for the 12 months ending in August of the prior year, then rounded to the nearest $1,000. The current tier amounts in the statute ($85,000 / $107,000 / $133,500 / $160,000 for individuals) are indexed against the 12 months ending August 2018. The $500,000 top threshold is excluded from indexing until 2028.

threshold(Y) = round_to_$1,000( base × avgCPI-U(Sep Y-2 … Aug Y-1) / avgCPI-U(Sep 2017 … Aug 2018) ) base avg (Sep 2017–Aug 2018) = 249.280 check: avg Sep 2024–Aug 2025 = 319.205 → ratio 1.2805 → $109k / $137k / $171k / $205k ✔ matches CMS 2026 2027: avg Sep 2025–Aug 2026 = 329.319 (Oct 2025 = 325.604) → ratio 1.3211 → $112k / $141k / $176k / $211k 11 published months only = 329.656 → ratio 1.3224 → $112k / $142k / $177k / $212k

Joint thresholds are exactly double the individual ones. The same formula reproduces the official 2020, 2025 and 2026 thresholds. One complication: the Bureau of Labor Statistics never published an October 2025 CPI-U (because of the 2025 lapse in appropriations), and SSA/CMS have not said how they will handle it. The first threshold ($112,000 / $224,000) is the same under every treatment; tiers 2–4 can differ by $1,000 (single) / $2,000 (joint). Our calculator uses the lower thresholds, which never understate a surcharge. Data: BLS series CUUR0000SA0.

The 2027 Part B standard premium and IRMAA dollar amounts depend on projected Medicare Part B spending and will be set by CMS. We do not guess them; we will update this page when they are published.

State of the CPI-U data for 2027

All months in the September 2025 – August 2026 window are final except October 2025, which BLS never published because of the 2025 lapse in appropriations. No further data can change the projection; only SSA's treatment of the missing month remains open.

Quick answers

What are the 2027 IRMAA brackets?

CMS has not published them yet. Using the statutory CPI-U formula, the first 2027 IRMAA threshold is projected at MAGI above $112,000 (single) or $224,000 (married filing jointly), based on your 2025 tax return. The top tier stays at $500,000 / $750,000, which by law is not indexed until 2028.

Which tax year is used for 2027 IRMAA?

Normally your 2025 federal tax return (the year two years before the premium year). If that return is not available, SSA may use your 2024 return and later correct it.

When will the official 2027 IRMAA amounts be announced?

CMS typically announces the next year's Part B premium and IRMAA amounts in the fall (the 2026 figures were released November 14, 2025). We will update this page once the 2027 figures are published.

What are the 2027 IRMAA brackets for married filing jointly?

Projected: above $224,000 joint MAGI (2025 tax year), with later tiers above $282,000–$284,000, $352,000–$354,000, $422,000–$424,000, and at $750,000. Each spouse on Medicare pays the surcharge separately.

What will the 2027 Part D IRMAA be?

Not yet known. Part D uses the same income tiers as Part B, so the projected thresholds apply to both, but CMS has not announced the 2027 Part D surcharge amounts. In 2026 they range from $14.50 to $91.00 per month.
